Fee: $109

Instructions:
Complete the information below online and click NEXT to confirm your registration. Submitting that form online will reserve your spot for two weeks, pending payment. After clicking SUBMIT on the following page, you will see a page confirming your reservation. To complete the process, print and sign the confirmation page and mail it with $109 payment to the address indicated. If the session you are interested in shows "full pending payments," please get on the waiting list. If a payment is not received within two weeks of online registration, the spot is immediately filled from the waiting list.

I understand that participation involves certain risks of injury and I agree that registration assumes these risks. I hereby release U.S. Baseball Academy, Midwest Baseball Academy, Inc., its directors and coaches from liability for such injuries. I understand that cancellations prior to 30 days before the start of the session are subject to a $50 refund. In the case of cancellation within 30 days of the first session, no refund will be given.
